Hospital/Homebound
Hospital/Homebound (HHB) Instruction provides academic support for students who are unable to attend school due to a medical condition, as certified by a licensed physician or psychiatrist. If a student is, or is expected to be, absent for ten or more consecutive school days, the parent or guardian must contact the Student Services Office to begin the HHB referral process.
The required HHB referral and medical release forms must be signed by both the physician and parent, then submitted to Student Services. Within five school days of receiving the completed paperwork, the school will schedule a meeting to determine eligibility and develop an Educational Service Plan. This plan may include accommodations through the Pyramid of Intervention, a Section 504 Plan, or an IEP, and will also outline a school reentry plan for the student’s return.
Please note that some students who have recurring or chronic health conditions may need to have a 504 plan or may need to be considered for Other Health Impaired services. The 504 plan or an IEP can address intermittent, chronic and recurring school absences without having to have a physician's referral each time. These cases will be handled differently than those that are one-time occurrences.
